patient informationBy blending neurology, pain management and physical therapy into one office we have successfully encompassed many of the needs of our patients in the same facility. We provide appropriate treatment plans designed to promote and improve the quality of health for our patients. This approach ensures clinical excellence, the personal touch and the quality care our patients have come to know and love.

New patients should arrive thirty minutes before their scheduled appointment. In order to expedite your appointment, please complete all forms prior to your arrival. We do our best to see all patients in a timely manner. However there are times when difficulties arise with patient care that may cause us to fall behind schedule. Should this be the case, we would appreciate your understanding. If the wait time causes a difficulty for you, please notify the Receptionist. We will be happy to reschedule your appointment.

Prescription Policy
PRESCRIPTION REFILL:
Please call at least seven (7) days in advance for a prescription refill.
Prescriptions will not be refilled on weekends or after normal business hours.

ABSOLUTELY NO EARLY REFILLS ON NARCOTICS OR SCHEDULED DRUGS.
Prescriptions are typically phoned-in to the pharmacy at the END of each clinic day.

Scheduling Policy
FOLLOW-UP PATIENTS:
Follow-up patients should arrive fifteen minutes before scheduled appointment.

NEW PATIENTS:
New patients should arrive thirty minutes before scheduled appointment.

Patient Instructions

Offering the latest therapies and techniques for our patients is a standard we insist upon. Most office services require no additional preparation for our patients. However, should you require any procedure listed, there are steps that must be taken prior to the procedure.
